Overview of Services

Chameleon MD provides low-cost, subscription-based and pay-per-visit telehealth medical services. Our licensed healthcare providers offer virtual consultations, assessments, and treatment recommendations as permitted by applicable state laws. Chameleon MD is not an emergency service. If you are experiencing a medical emergency, call 911 immediately.

Eligibility

To use our Services, you must: 

• Be at least 18 years of age or the age of legal majority in your state, 
• Provide accurate, current information, and 
• Physically be located in a state where Chameleon MD operates and where our providers are licensed.

 Minors may use the Services with a parent or legal guardian’s consent, as permitted by state law.

Medical Disclaimer

Our providers make medical decisions based on the information you provide. You agree that: 

• You will provide truthful, complete, and accurate medical history and information. 
• Telehealth has limitations, and in some cases, an in-person evaluation may be recommended. 

Chameleon MD does not guarantee diagnoses, treatment outcomes, or the availability of medications.

Not for Emergencies

Chameleon MD is not suitable for: 

• Emergency medical needs 
• Life-threatening symptoms 
• Severe or uncontrolled conditions 

If you experience any of the above, call 911 or visit the nearest emergency department.

Memberships, Fees, and Billing

Chameleon MD offers: 

• Pay-per-visit non-member care 
• Individual annual membership plans
• Family annual membership plans 
• Company/Business annual membership plans 

All fees will be clearly displayed at checkout. Memberships continue until canceled. You authorize Chameleon MD to charge your payment method on a recurring basis unless you cancel. No refunds will be issued for partial billing periods.

No Insurance Billing

Chameleon MD does not accept or bill insurance. You are responsible for all costs associated with your use of the Services.

Privacy & Security

Your personal and medical information is protected under HIPAA and our Privacy Policy. By using the Services, you consent to our collection and use of data as described in the Privacy Policy.

Prescription Policy

Our providers may prescribe medications when clinically appropriate. We do not prescribe controlled
substances, lifestyle drugs, or any medication prohibited by telehealth regulation.

Prescription availability depends on:

• Provider judgment
• Applicable state laws
• Pharmacy fulfillment policies

Chameleon MD is not responsible for pharmacy delays or refusals.

User Responsibilities

You agree to: 

• Use the Services only for lawful and personal medical purposes 
• Follow provider instructions 
• Not misuse or interfere with the platform 
• Maintain your own device, internet access, and security

Cancellation & Termination

You may cancel your membership at any time through the platform. Cancellation takes effect at the end of the current billing cycle. 

Chameleon MD may suspend or terminate your account for: 

• Violations of these Terms 
• Fraudulent activity 
• Misuse of Services 
• Abuse toward providers or staff

Limitations of Liability

To the maximum extent allowed by law, Chameleon MD is not liable for: 

• Any indirect, incidental, or consequential damages 
• Loss of data 
• Issues caused by user-provided inaccurate information 
• Delays or service interruptions outside our control 

Your sole remedy for dissatisfaction with the Services is to discontinue use.

No Warranty

All Services are provided “as is” without warranties of any kind. We do not guarantee availability, accuracy, or uninterrupted access.

Contacting Your Provider

Chameleon MD offers secure messaging and follow-up options within the portal. Response times may vary, but we strive to address needs promptly.

Changes to Terms

We may update these Terms at any time. Continued use of the Services constitutes acceptance of the revised Terms.

Governing Law

These Terms are governed by the laws of the state in which Chameleon MD is headquartered, without regard to conflict-of-law rules.
