By choosing to receive care through ChameleonMD’s telehealth services, you acknowledge and agree to the following:

Telehealth uses secure electronic communications to connect you with a healthcare provider. This may include video calls, phone calls, messages, or electronic sharing of medical information.
- Technology Used:
ChameleonMD uses HINT, a secure, HIPAA-compliant telehealth platform, to deliver care and protect your privacy.
- Potential Risks:
- Technology may fail or be interrupted.
- In rare cases, unauthorized access or technical failures could affect confidentiality.
- If a connection fails, we will make reasonable efforts to reconnect or offer an alternative.
- Benefits:
- Convenient access to medical care
- Reduced travel and wait times
- Ability to receive ongoing care even when in-person visits aren’t possible
- Confidentiality:
Your information will be protected according to HIPAA standards. All communications, recordings, and shared data are encrypted and stored securely. ChameleonMD will not record or distribute your telehealth sessions without your consent.
- Patient Rights:
- You may withdraw your consent for telehealth at any time without affecting future care or treatment.
- You have the same rights and responsibilities as with in-person visits, including privacy and access to your records.
- Your Acknowledgment: By participating in a ChameleonMD telehealth visit, you acknowledge that you:
